A Care Plan starts with a competent health care team. 

 Get a good primary care doctor to coordinate your team.  Organize and research your parent's health records. Learn about their meds and how you can help them use them properly. Know the specifics about their condition.
 

  • You can check with your parents' State Medical Board to verify the license credentials of all their doctors.
  • Keep updating the names of the nearest hospitals and emergency care facilities in their neighborhood.
  • Learn the hospitals' HIPAA policy to release medical information. Make sure you get the permission required.
  • Know how to research specific medical conditions.
  • Check our Meds and pharmacy to make sure your parents understand what they are taking.
